Notifiable Events - Information for service providers
Registered Establishments and Agencies are required by legislation to report specified adverse events to the RQIA. Following extensive consultation with providers, the RQIA has developed a revised process for reporting these events using newly developed pro formas. Establishments and Agencies need to notify the RQIA in accordance with the statutory regulations governing their services. Whilst the regulations vary in terms of the exact time scale for reporting, written notifications should be sent to RQIA no later than within 3 working days of the notifiable event.
The following documents are available below:
- Guidance document which outlines the specific details of the Reporting Arrangements
- Guidance on legislative reporting requirements (A3 document)
All notifications from services in relation to adverse incidents and deaths should made using RQIA's reporting templates as outlined below.
- Form 1a: Initial Notification - Adult Services
- Form 1b: Initial Notification - Children's Services
- Form 2: Follow up Notification - All Services
- Form 3: Service Declaration - All Services
Completed notification forms should be returned by:
- Email: incidents@rqia.org.uk
- Fax: (028) 9051 7590
In exceptional circumstances, where technical difficulties make it impossible to submit a notification by either email or fax, providers can continue to post their notifications to RQIA. Notifications should be clearly marked as Private and Confidential and sent to:
