The GP/medical practitioner may, following an assessment of the patient mental state and consideration of any substantial risks of serious physical harm to the patient or others that might exist, conclude that a medical recommendation should be made.

The GP/medical practitioner may wish to consult with the ASW or nearest relative before making a final decision in relation to the medical recommendation. As stated previously the Code states that it is good practice for the professionals involved in the application for admission to be present at the same time, although it may be advantageous for each to interview the patient separately. Code 2.5

The GP/medical practitioner should consult with and advise the doctor in the hospital who has requested the medical recommendation and nursing staff of the outcome of the medical examination and decision to complete Form 3.
