The nearest relative must have seen the patient within 2 days prior to the completion of their application. The nearest relative should be offered assistance and advice from the medical practitioner and other professionals involved.
The GP/medical practitioner should advise that the advice and assistance of an ASW is available 24 hours each day in relation to both the application and any necessary conveyance arrangements.
