On 11 August 2022, the Chief Medical Officer wrote to RQIA commissioning an independent Review of the Southern Health and Social Care Trust (SHSCT) Urology Services, and a review of the “Lookback” exercise relating to patients previously under the care of a consultant, Mr O’ Brien.
This work was undertaken by RQIA in two phases:
- RQIA Review of Southern HSC Trust Urology Services and Lookback Review, which produced a report in July 2023; and
- Southern HSC Trust Urology Services Phase II Report. Governance and Assurance Arrangements & Patient Views and Experience, which produced a report in August 2024.
Mindful of the need to fully respect the independent Urology Services Inquiry process, the final reports were shared with the Inquiry for their consideration but withheld from publication at that time.
As the independent Urology Services Inquiry has now completed its work and published its Report. The Authority believes it is in the interests of transparency to place this information on the public record now.
The implementation of the recommendations arising from these Reviews falls to the SHSCT reporting to SPPG in the Department of Health.
